Preschool/4k Enrollment Packet
We are a preschool, 4K, after-school and summer camp program in Madison, WI. We serve children ages 3 (and potty trained) through age 11. Our pursuit is to emphasize equality, solidarity, emotional literacy, and exploration of the arts within our classrooms and surrounding community. It is our goal to build on a child’s academic foundation with play-based learning, hands-on experimentation, group discussions and use of local resources as an extension of our classroom environments. UADC is committed to working cooperatively with all families to foster an interwoven community that supports and enriches our shared experiences. We provide individualized, child driven programming and support for families in the heart of Madison.
University Avenue Discovery Center Preschool is licensed by the Wisconsin Department of Children and Families and is accredited by the National Association for the Education of Young Children (NAEYC). UADC is an equal opportunity provider.
UADC provides developmentally appropriate early childhood care and education. We participate in the USDA Child and Adult Care Food Program. UADC accepts tuition reimbursement grants from Dane County Human Services, University of Wisconsin CCTAP, and selected federal grants.
UADC is part of Madison Madison Metropolitan School District’s 4 year-old-kindergarten program. Students must be enrolled with UADC and MMSD to attend 4K. Enrollment with MMSD can be done on-line at http://deelweb.madison.k12.wi.us/4k .

Fees/Registration
A non-refundable 4-week tuition deposit is due at the time of enrollment (fee waived for all families that receive needs-based funding from outside agencies). This deposit may be broken up into three equal payments and paid over three months. The registration fee is applied to the last month of your child’s enrollment contract. A waiting list is maintained in the order that applications are received. The first week’s tuition is due at the start of your child’s first week of care.
Parents have the option of signing a 12-month, 9-month or a monthly contract. Tuition rates reflect the type of contract signed. Families withdrawing their children prior to the completion of the contract date forfeit the registration fee as a penalty for early withdrawal and breach of contract. Parents who sign a 12 month contract agreement are allowed a one-week vacation (5 consecutive days) of their choosing within the contract period, free of tuition payment. This does not apply to 9 month or monthly contract agreements.
Tuition
Tuition and fees are on a monthly basis. Payments are due on or before the first attendance day of the period for which tuition is paid. Tuition that is paid by the month is due on the first of each month. Parents have the option to pay by check, money order or credit/debit card.
UADC accepts tuition rates for subsidized parents set by public agencies at the highest allowed rate. Parents are responsible for parent share-payments. Scholarships are available for qualifying families.
There is a 10% discount on one child for families that are enrolling more than one child at UADC.
Final Enrollment Procedures
After submitting the application and deposit, parents receive a confirmation of their registration and a copy of the parent handbook. It is expected that parents will become familiar with and agree to all policies. Parents are encouraged to discuss any policy questions with the director.
At least 3 weeks prior to the date of admission, or on receipt of the application and registration fee, parents will be sent final enrollment materials. Parents are offered a parent-teacher conference at or before the time in which their child(ren) enroll. Parents are required to return all forms one week prior to enrollment.

Parking
Parking is available for drop-off, pick-up and visits in the city designated 10 minute loading zone on the east side of Lathrop Street as well as the upper lot.
